If you are new to our practice and have an unusual eye disease and/or wear custom contact lenses, please complete this form and give it to your previous eye doctor in advance of your appointment. This will help us make the continuity of your eye care as seamless as possible. For most of our new patients, the Transfer of Records is not necessary.
